cu scopul de a realiza o mai bun\u0103 conectare a nivelurilor de \u00eengrijire \u00eentre ele \u0219i de a furniza personalului medical instrumentele necesare, astfel \u00eenc\u00e2t femeile \u0219i copiii rom\u00e2ni, dar \u0219i cei refugia\u021bi \u00een \u021bara noastr\u0103 s\u0103 beneficieze de servicii medicale complete, sigure, u\u0219or de accesat \u0219i echitabile.<\/p>\n<p>Thus, between\u00a0<em>septembrie 2022 &#8211; martie 2023<\/em>with the support of UNICEF in Romania, our association delivered in\u00a0<em>six cities<\/em>\u00a0in Romania (<em>(Bucharest, Constan\u021ba, Bra\u0219ov, Vaslui, Suceava, Ia\u0219i<\/em>) o serie de cursuri de preg\u0103tire pentru medici, moa\u0219e, asisten\u021bi medicali din \u00eengrijirea primar\u0103 \u0219i maternit\u0103\u021bi &#8211; s\u0103 lucreze direct cu femeile \u0219i copiii, inclusiv refugia\u021bi \u00een Rom\u00e2nia, fiind\u00a0<em>the first initiative of its kind that brings together medical professionals from primary, secondary and tertiary care.<\/em>.<\/p>\n<p>During this period,\u00a0<strong>250 health professionals<\/strong>\u00a0din ora\u0219ele men\u021bionate &#8211; medici \u0219i asisten\u021bi medicali din cabinete de medicin\u0103 de familie, moa\u0219e \u0219i medici reziden\u021bi \u0219i asisten\u021bi medicali de obstetric\u0103-ginecologie \u0219i neonatologie &#8211; au beneficiat de formare bazat\u0103 pe cele mai noi \u0219i eficiente practici privind \u00eengrijirea antenatal\u0103, dar \u0219i cu privire la alfabetizarea \u00een domeniul s\u0103n\u0103t\u0103\u021bii, m\u0103suri de s\u0103n\u0103tate public\u0103, educa\u021bie postnatal\u0103, consiliere \u0219i sprijin \u00een domeniul al\u0103pt\u0103rii, \u00eengrijire medical\u0103 \u0219i preven\u021bie pentru nou-n\u0103scu\u021bi \u0219i copii, promovarea vaccin\u0103rii pentru popula\u021bia pediatric\u0103 din Rom\u00e2nia, inclusiv cea refugiat\u0103, \u0219i combaterea violen\u021bei sexuale \u0219i de gen.<\/p>\n<p>The training sessions were structured in three sessions of four modules each and were delivered by eight specialists - midwives, neonatologists, health literacy experts, psychologists, breastfeeding (IBCLC), public health, and sexual and gender-based violence consultants. In addition to their academic and professional training, the trainers who taught the courses also benefited from specialist training to develop skills in facilitating and promoting services for mothers and children, including refugees in Romania.<\/p>\n<p>As a result of the debates and active participation in the courses, the participating health professionals and the eight specialists reiterated or identified a number of problems faced by pregnant women and medical staff on all these topics, as well as solutions that will help to solve them.<\/p>\n<p>Indiferent de ora\u0219ul \u00eentre medicii din diferite specialit\u0103\u021bi, asisten\u021bi, moa\u0219e, lipsa educa\u021biei \u0219i a alfabetiz\u0103rii pentru s\u0103n\u0103tate, a cursurilor de formare continu\u0103 pentru personalul medical, de comunicare personal medical &#8211; pacient, precum \u0219i nevoia de debirocratizare \u0219i degrevare a personalului medical pentru a putea acorda mai mult timp pacientului.<\/p>\n<p>One of the main benefits appreciated by our course participants has been that this is the first time that such specialists have been brought together in a training program dedicated to primary care and hospital medical teams.<\/p>\n<p><strong>In terms of access to antenatal care services,<\/strong>the following problems faced by health care staff or women with direct consequences on the health of pregnant women, mothers and babies emerged from the course sessions, discussions and feedback:<\/p>\n<ul>\n<li><strong>Access to antenatal care is severely restricted, the main reasons being budgetary, time-related and determined by lack of qualified staff and dedicated programs;<\/strong><\/li>\n<li>Apart from Bucharest, there are no specific antenatal education programs, carried out according to a uniform national program, to which mothers have access free of charge; in rural areas of the regions where the courses have been held there are no qualified staff to provide antenatal education;<\/li>\n<li>Access to antenatal care for pregnant women in rural\/disadvantaged areas is restricted due to lack of qualified staff or family doctors, and mothers cannot travel to other cities for follow-up due to lack of financial resources;<\/li>\n<li>It is difficult for medical staff to provide antenatal education because discussions about medical conditions or best practices have to start from scratch during the consultation in too short a time; for hospital staff, the time spent with the mother in the maternity ward is not enough to prepare parents for the postpartum period and life with the newborn;<\/li>\n<li>The lack of midwives in the system and the ability to provide the full set of services for women reduces access to antenatal care;<\/li>\n<li>Lack of funding for investigations and non-payment by the state of certain antenatal care services (e.g. morpho-fetal ultrasound, complex tests);<\/li>\n<li>Lack of campaigns to educate and promote the role of family doctors and midwives in antenatal care;<\/li>\n<li>Difficulties in navigating the health system and accessing health services for refugee women, language barrier and excessive bureaucracy in the procedure for taking refugees into the care of family doctors; reporting problems to the National Health Insurance House and errors in the Single Integrated Information System;<\/li>\n<li>Lack of patient communication courses and training courses on dealing with pregnant minors.<\/li>\n<\/ul>\n<p>Following the interaction of trainers with medical professionals in training, evaluation and feedback sessions, in order to address some of the needs of medical staff so that services are patient-centered, the following solutions and directions for action were identified:<\/p>\n<ul>\n<li>Creation of education and information programs for mothers and creation of dedicated funding lines for antenatal care;<\/li>\n<li>Preparation and continuous training of health professionals for the provision of antenatal care based on guidelines, evidence-based practice, collaboration and communication between specialties in the field of maternal and child care;<\/li>\n<li>Campanii de educare \u0219i promovare a rolului medicului de familie \u0219i moa\u0219ei \u00een \u00eengrijirea antenatal\u0103, a Carnetului gravidei \u0219i a colabor\u0103rii multidisciplinare medic de familie &#8211; obstetrician &#8211; moa\u0219\u0103;<\/li>\n<li>Cursuri de comunicare personal medical &#8211; pacient, axate pe empatia fa\u021b\u0103 de pacient \u0219i pe implicarea acestuia \u00een actul medical, respectiv colaborarea \u00een re\u021bea \u0219i echipe pluridisciplinare pentru a acoperi nevoile gravidelor \u0219i cuplurilor;<\/li>\n<li>Correct legislation for universal access, so that midwives can provide the full set of services to pregnant women, mothers and couples and carry out their activities autonomously, as required by law, unconditionally or delegated to other medical staff;<\/li>\n<li>De-bureaucratize procedures and free up medical staff to give more time to the patient;<\/li>\n<li>Free courses for future parents and involvement of midwives in antenatal education; free courses organized in maternity wards;<\/li>\n<li>Social programs focusing on specific community problems, involving midwives in antenatal care so that disadvantaged communities without access to obstetricians and far from urban centers can be served;<\/li>\n<li>Dedicated programs for medical professionals on collaboration, teamwork and communication.<\/li>\n<\/ul>\n<p><strong>Feedback personal medical &#8211; Re\u021beaua \u201eGrij\u0103 de Mam\u0103\u201d\u00a0<\/strong><\/p>\n<p>Feedback received from health care staff is a critical component in learning about the challenges and issues patients and health care professionals are facing and also in helping us identify solutions so that health care professionals can provide comprehensive, safe, accessible and equitable services for pregnant women, mothers and children.<\/p>\n<p>The results of the feedback questionnaires received from 76.8% of the participants in the \u201cCourses to improve access to healthcare and support for mothers and children\u201d sessions of the \u201cCaring for Mothers\u201d Network were centralized and the following data emerged:<\/p>\n<p>Regarding\u00a0<strong><em>Solutions for improving access to antenatal care for Romanian and refugee women<\/em><\/strong><em>,\u00a0<\/em>among the respondents to the feedback questionnaire:<\/p>\n<\/div>\n<div id=\"sp-f08qj3\" class=\"sp-flex sp-css-target sp-el-block sp-bulletlist-block-template-0 sp-justify-start sp-bulletlist-block-f08qj3\">\n<ul class=\"sp-bulletlist-wrapper\">\n<li class=\"sp-flex sp-items-center sp-bullet-list-item-f08qj3-0 sp-justify-start\"><i class=\"fa-fw fa-lg fas fa-arrow-circle-right\"><\/i><strong>82,29%<\/strong>\u00a0consider\u0103 necesare serviciile medicale publice de \u00eengrijire antenatal\u0103 accesibile, decontate din Fondul Na\u021bional Unic de Asigur\u0103ri Sociale de S\u0103n\u0103tate &#8211; FNUASS (ex.: examene medicale, screeninguri antenatale, investiga\u021bii paraclinice etc.);<\/li>\n<\/ul>\n<\/div>\n<div id=\"sp-lnbyzf\" class=\"sp-flex sp-css-target sp-el-block sp-bulletlist-block-template-0 sp-justify-start sp-bulletlist-block-lnbyzf\">\n<ul class=\"sp-bulletlist-wrapper\">\n<li class=\"sp-flex sp-items-center sp-bullet-list-item-lnbyzf-0 sp-justify-start\"><i class=\"fa-fw fa-lg fas fa-arrow-circle-right\"><\/i><strong>82,29%<\/strong>\u00a0consider that there is a need for education campaigns on the role of the family doctor and midwife in monitoring pregnant women;<\/li>\n<\/ul>\n<\/div>\n<div id=\"sp-d210q9\" class=\"sp-flex sp-css-target sp-el-block sp-bulletlist-block-template-0 sp-justify-start sp-bulletlist-block-d210q9\">\n<ul class=\"sp-bulletlist-wrapper\">\n<li class=\"sp-flex sp-items-center sp-bullet-list-item-d210q9-0 sp-justify-start\"><i class=\"fa-fw fa-lg fas fa-arrow-circle-right\"><\/i><strong>77,6%<\/strong>\u00a0think that midwives should be involved in public antenatal care services;<\/li>\n<\/ul>\n<\/div>\n<div id=\"sp-grvm7w\" class=\"sp-flex sp-css-target sp-el-block sp-bulletlist-block-template-0 sp-justify-start sp-bulletlist-block-grvm7w\">\n<ul class=\"sp-bulletlist-wrapper\">\n<li class=\"sp-flex sp-items-center sp-bullet-list-item-grvm7w-0 sp-justify-start\"><i class=\"fa-fw fa-lg fas fa-arrow-circle-right\"><\/i><strong>70,83%<\/strong>\u00a0consider that there is a need for accessible antenatal education modules paid for by the UNFUASS;<\/li>\n<\/ul>\n<\/div>\n<div id=\"sp-d4wd5l\" class=\"sp-flex sp-css-target sp-el-block sp-bulletlist-block-template-0 sp-justify-start sp-bulletlist-block-d4wd5l\">\n<ul class=\"sp-bulletlist-wrapper\">\n<li class=\"sp-flex sp-items-center sp-bullet-list-item-d4wd5l-0 sp-justify-start\"><i class=\"fa-fw fa-lg fas fa-arrow-circle-right\"><\/i><strong>2,60%<\/strong>\u00a0others (sex education classes in schools, lactation and breastfeeding education in schools, free services for minors regarding contraception, regulation of the midwifery profession).<\/li>\n<\/ul>\n<\/div>\n<div id=\"sp-pk17nu\" class=\"sp-css-target sp-text-wrapper sp-el-block sp-text-block-style sp-text-wrapper-pk17nu\">\n<p>Regarding\u00a0<strong><em>olutions for increasing access to antenatal education<\/em><\/strong>, respondents to the feedback questionnaire proposed:<\/p>\n<\/div>\n<div id=\"sp-jzblsl\" class=\"sp-flex sp-css-target sp-el-block sp-bulletlist-block-template-0 sp-justify-start sp-bulletlist-block-jzblsl\">\n<ul class=\"sp-bulletlist-wrapper\">\n<li class=\"sp-flex sp-items-center sp-bullet-list-item-jzblsl-0 sp-justify-start\"><i class=\"fa-fw fa-lg fas fa-arrow-circle-right\"><\/i><strong>85,93%<\/strong>\u00a0&#8211; Derularea de programe de tip \u201e\u0218coala p\u0103rin\u021bilor\u201d \u00een comunit\u0103\u021bile defavorizate;<\/li>\n<\/ul>\n<\/div>\n<div id=\"sp-epa7h3\" class=\"sp-flex sp-css-target sp-el-block sp-text-wrapper-szsk38\">\n<p>We were pleased to see in this project such a high level of commitment from medical professionals to work together to find solutions to provide better and more accessible healthcare for women and children<\/p>\n<p>Therefore, in the coming period, we will organize a series of meetings at central and local level with decision-makers and representatives of the authorities \u201cSupport Network for Reproductive Health and Increased Access to Pediatric Care for Refugee Women and Children\u201d, implemented with the support of UNICEF in Romania, medical services for refugee women and mothers from Ukraine, consultations for newborns, pre- and post-natal education.
